Liposuction

Running, jogging, skipping they say helps you lose weight. But there are certain bulges that no amount of dieting or exercise can get rid of. Eventually people opt for liposuction. This plastic surgery helps remove that extra fat without too much trouble, since today liposuction is a commonplace surgery.

For the last five years, lipoplasty, as liposuction is now medically termed, has been the leading cosmetic surgery procedure in this country, with 385,000 procedures last year. People 35 to 50 have the most procedures. Women represented a vast majority, but men made up 20 percent of the total last year, according to the American Society for Aesthetic Plastic Surgery.Most patients have an average of five pounds removed. Over the last several years, the operation has become safer, in part because smaller amounts of fat are removed. Most people can return to work within a week and begin light exercise in two weeks or so. Swelling usually goes down in about a week; bruising and some numbness generally last about three weeks.

The risk of death, which in 1998 was 1 in 5,000, dropped to 1 in 47,415 from 1998 to 2000, according to results of more than 94,000 procedures reviewed in the March/April 2001 issue of Aesthetic Surgery Journal. Surgeons' reports suggest complications have dropped as well; in the survey, the most common minor side effect was postoperative nausea or vomiting, while the most frequent major complication was skin peeling around the treated area.

Surgeons no longer routinely combine lipoplasty with other major surgery, says Dr. Charles Hughes III, a plastic surgeon in Indianapolis and the author of the journal article.

Fillers

An injection that banishes wrinkles for life, it sounds too good to be true. But there are some experts who believe that the new so-called ‘permanent filler’ can do just that. "It's not good stuff," said Dr Richard Ellenbogen, a Los Angeles cosmetic plastic surgeon, who says he won't offer the new filler. "It's very tricky to use," he said. "It causes granulomas, those unsightly bumps, in the lips and nasolabial folds."Though there are doctors who differ on these fillers.

There are some physicians look on ArteFill as one more valued tool to banish wrinkles. "If it is placed properly, there are no granulomas," said Dr.Marian Northington, an assistant professor of dermatology at the University of Alabama at Birmingham.

She noted that other fillers entail risks for bumpy skin scars, too.She and Ellenbogen offered up differing views on ArteFill after the injection became the hot topic at the recent annual meeting of the American Academy of Dermatology, in San Diego.

Banishing wrinkles with so-called "soft tissue fillers" such as Restylane or collagen can be a high-maintenance, expensive proposition. Even the longest lasting fillers typically dissipate after six months to a year or so, warranting another trip to the cosmetic surgeon or dermatologist.

Doctors attending the AAD were clearly divided when it came to ArteFill. Some said they'd use it liberally, while others said they won't touch the stuff because the risk of bad results isn't worth it.

Plastic surgeons nationally and locally say that several factors are behind the trend: Buoyed by such TV shows as "Extreme Makeover" and "The Swan," overall interest in plastic surgery has grown. Demographic shifts have rendered the population more diverse. Minorities have become more affluent, giving them the extra income to spend on such procedures.

Cosmetic surgery "is not so much a taboo in the ethnic consumer market anymore," says Dr. David Watts, a Philadelphia-area plastic surgeon, who is developing a skin-care line customized for minority patients. "Now it has become so mainstream, it's just considered a fact of life.

"In Whittaker's case, the nose job wasn't a big deal. She likes her reflection more, but others don't notice a dramatic change. If she runs into someone she hasn't seen for a while, they don't comment on her nose but say just that she looks good. "You can tell a difference, but it's so subtle I still look totally like myself," says Whittaker.

Nose reshaping is the most commonly requested procedures for Asian-Americans and blacks, according to the American Society of Plastic Surgeons. Breast augmentation leads the pack for Hispanic patients.

In 2006 there were more than 2.5 million cosmetic surgery procedures performed on non-Caucasian patients, accounting for 23 percent of all plastic surgery patients. As plastic surgery has gained acceptance among minorities, the ideas of attractiveness has changed, says Watts. Women such as J.Lo, Lucy Liu and Beyonce are now all considered among the fairest in the land.

"It's no longer just this Nicole Kidman look that spans everything for everyone," Watts says. "People are really looking at what's considered beautiful, and I think it's starting to cross all of these ethnic borders."

Neck Lift
